Ipamorelin is a five-residue synthetic peptide (Aib-His-D-2-Nal-D-Phe-Lys-NH₂) that Novo Nordisk chemists reported in 1998 as an agonist of the growth hormone secretagogue receptor — the receptor cloned in 1996 and later shown to be ghrelin’s^{[1, 2]}. Its published record is largely rat and swine pharmacology, with one human volunteer pharmacokinetic study and one randomised trial that did not beat placebo^[7]. What researchers have looked at

• Hormone selectivity (swine, 1998): GH release comparable to GHRP-6 with no rise in ACTH or cortisol above the GHRH response, and no change in FSH, LH, prolactin or TSH^[1].
• Pharmacokinetics (rat, 1998): five-fold lower clearance than GHRP-6, excretion mainly in urine, 60–80% recovered as intact peptide^[3].
• Skeleton (adult female rats, 1999–2000): dose-dependent increase in tibial longitudinal growth rate over 15 days^[4]; after 12 weeks, higher bone mineral content that vanished once corrected for body-weight gain^[5].
• Body fat (mice, 2001): relative fat mass, leptin and food intake went up, through GH-independent mechanisms — the opposite direction to GH itself^[6].
• Humans (2014): a 117-patient randomised trial in postoperative ileus found no significant difference from placebo on any efficacy endpoint^[7].
